 Directions Consider health statistics and demographics; create a plan of care for your vulnerable population. My vulnerable population is IMMIGRANTS Peer-reviewed references, not older than 2014. Criteria 1. Epidemiological data of population is foundational to the plan of care that is created. 2. Implementation strategies include the role(s) of the CHN and three levels of prevention. 3. Empowerment concepts are included in plan and implementation. 4. Existing resources are described and case management as a potential tool is discussed. 5. Paper is well-organized and well-developed; reasoning is substantial and logical. Paper reflects proper word choice, sentence structure, spelling and punctuation. APA format is utilized.
